Mission bug: When using the N3 Burst Rifle with the Venom ability, obtained two points for kills with the SWARM gun. This issue may have something to do with getting assists, or others assisting my kills with the SWARM gun equipped to themselves.
Minor bug: When game starts, countdown sits at 0, then starts approximately 3 seconds later.
Menu bugs: At one point, I joined a match in progress, and the game wouldn't allow me to choose a loadout before entering. It went to the loadout screen, but each choice was blank. It also wouldn't display the game mode or map. Only happened once.
Very minor, but every now and then the music cuts out in the various menus.
Cover: In Drydock, sometimes when I hide behind the cover on the side walls, my vision will be pulled in some random direction or towards the ceiling.
Thanks for these reports guys. I'll try to mark them viewed when we have people on them.
BTW, thank you to BerzerkCommando for the pictures. If you guys can take pictures, even just cell phone captures, it's really beneficial for identifying bugs!
Looks like the Hybrid servers are down, time to report some bugs.
After a game finished and I pressed A to go back to the region+disctrict selection I got this screen instead.

Not a problem I just pressed A and went to the map screen.
I noticed that when people enter a game, vote for a map/gametype and leave that their votes don't disappear.

See how there are 6 votes but only 4 players. After this some more players joined and the votes went up to 7.

Gained experience/levels sometimes lost after exiting score/stats screen. Confirmed (5+ times) that this happens when the World Data syncs right after I exit a game. I've already lost quite a few levels/guns this way. This needs to be fixed immediately because people will not play if the game cheats them out of their earnings.
When you die shortly after using your ability, the cooldown graphic disappears (i.e., the icon stops filling). This needs to be fixed; although, I think that when you die, your cooldown should be reset anyway.
Firing Accuracy is rarely correct. I had 10 kills and 4 headshots in one match, but my reported firing accuracy was 0%.
South America district 8 has no Mission.
Asia district 8 has no Mission.
Europe district 8 had a Mission, but no longer has one.
Africa district 16 has no Mission.
The music always cuts out when the Loading circle fills halfway yellow.

I'm a level 29. I got enough EXP in a game to get to level 30. After the match it showed me getting to level 30. But when I got back to the map screen I'm back to level 29. With the same EXP points I had before that match.

I experienced a nearly game-crippling glitch in which most of the elements to my HUD were gone including; crosshair, ability symbol, gun symbol + ammo. I also had game geometry appear on my screen occasionally along with strange colors and graphics when taking damage and upon death. The last thing that I noticed was that all "extra" effects were gone - things like muzzle flash, bullet trails, grenade explosions and jet-pack exhuast. This glitch continued throughout the entire match, regardless of what I did.

A host dropped right as the game mode and map were chosen, but I wasn't sent back to the world map. Instead I was sent to the loadout menu, in which I was stuck because the match was no longer officially in progress and nothing would load. I couldn't exit out of the match either so I had to go to the xbox home in order to get out.
